简介： 自从家庭护理从区域责任转移到市政责任，并且自新的2018年医院出院法规实施以来，瑞典的医疗保健经历了重大变化。这导致作业治疗师（OTs）的工作方式发生了改变，因为OTs在规划患者出院方面扮演着关键角色，这是一个至关重要的过程，在此过程中患者将返回家中，并且在这个过程中OTs还涉及到护理提供者之间的过渡。
目的： 本研究的目的是阐明作业治疗师在住院护理和家庭护理之间出院过程中的专业内合作体验。
方法： 与来自瑞典三个地理区域的作业治疗师（N = 12）进行了访谈，其中住院护理和家庭护理的比例均衡。数据分析根据定性内容分析进行。
结果： 贯穿所有类别的一个中心主题是：专业内沟通具有多面性、交织性和情感性。这些类别揭示了关于患者参与度的重要但被忽视的安全问题，不同场景下的思维模式差异，复杂的快速沟通，不可预测的出院路径，责任和组织指南不明确以及对职业健康和作业治疗师情绪的影响。
结论： 专业内沟通非常复杂，有许多方面和视角对于合作能够顺利进行至关重要。这也给参与协作过程中的每一个人带来了高要求，以区分并识别确保患者及其家属、相关服务以及对OTs来说可管理的出院安全所必需的信息。
