Materials & methods: This retrospective cross-sectional study was conducted on all pediatric patients with posterior fossa tumors admitted to teaching hospitals affiliated with Isfahan University of Medical Sciences from 2017 to 2022....Forty-three patients with posterior fossa tumors were identified, and seven patients were excluded due to diagnoses other than medulloblastoma or ependymoma.

Indications for EVD included posterior fossa tumor surgery (n=39), shunt infection and intracerebral or intraventricular hemorrhage (n=22), and intracerebral abscess (n=1).

Following posterior fossa decompression, the patient experienced significant neurological improvement, including restored motor function and resolution of intracranial edema, although the cervical syrinx cavity remained stable.

Results: Clinical presentations depend on the compression location, such as the "talk and die" phenomenon in posterior fossa AEDH or motor impairments due to subfalcine herniation.

Objectives: To evaluate pediatric patients with posterior fossa tumors for incidence, clinical characteristics, pathophysiology, risk factors, and neuroradiographic features of this syndrome....Materials and methods: The study included 60 pediatric patients with a posterior fossa tumor who underwent surgery by a telovelar approach. Detailed pre- and postoperative clinical and radiological evaluations were done....fossa structures.Pathological-histopathology of tumor. The preoperative, immediate postoperative, and 1-year postoperative imaging results were reviewed to assess the neuroradiographic features in the two groups....The presence of brainstem and peduncular edema in the immediate postoperative period ( p -value = 0.04) and gross atrophy of posterior fossa structures at 1 year ( p -value = 0.01) showed significance toward the development of CMS....Keywords: dysarthria; mutism; neurobehavioral; posterior fossa syndrome; posterior fossa tumors in children. Pediatric posterior fossa tumors, particularly astrocytomas, pose significant diagnostic challenges due to their histological diversity and overlapping imaging features. Conventional methods relying on radiologists' interpretations are prone to subjectivity and variability....Studies were included if they evaluated neural network-based AI for pediatric posterior fossa tumor diagnosis and reported quantitative performance metrics....Keywords: artificial intelligence; diagnostic accuracy; neural networks; pediatric astrocytoma; posterior fossa tumors. Patients underwent one of two surgical approaches: posterior fossa decompression (PFD) or posterior fossa decompression with duraplasty (PFDD). Data on clinical symptoms, intradural findings, surgical outcomes, and postoperative complications were collected and analyzed.

They were both successfully operated on due to the posterior fossa dysplastic gangliocytoma, which was histopathologically confirmed as Lhermitte-Duclos disease....Surgical posterior fossa decompression is required at the onset of neurological symptoms to relieve the mass effect and provide tissue samples for further analysis, ensuring a favorable outcome.

It often presents with nonspecific neurological symptoms and can mimic other posterior fossa tumors, such as medulloblastoma, on imaging....Case description: We present the case of a 10-year-old girl who developed progressively worsening neurological symptoms, raising suspicion of a posterior fossa tumor. Initial magnetic resonance imaging findings suggested a diagnosis of medulloblastoma....Conclusion: This case highlights the importance of considering DMG, particularly with H3K27 alterations, as a differential diagnosis in posterior fossa tumors. The presence of these genetic mutations significantly impacts both treatment decisions and prognosis.
