简介： 急性肾损伤（AKI）是重症监护病房（ICU）患者常见的并发症，具有较高的发病率和死亡率。

目的： 分析连续肾脏替代治疗使用柠檬酸钠作为区域抗凝剂时机械性和代谢性不良事件的发生率。

材料与方法： 本研究为一项为期三年的观察性、描述性的前瞻性研究，在一个多学科ICU进行。纳入标准包括患有急性肾损伤并接受肾脏替代治疗和柠檬酸钠（Prismocitrate 18/0 mmol/L [0.5%]）治疗的患者。排除标准包括肝功能衰竭、活动性出血、严重血小板减少症、ICU停留时间少于24小时或使用其他抗凝剂的患者。记录人口统计学变量、严重程度指数（APACHE II）、血管活性药物使用情况、不良事件和导管特征。通过滤器持续时间评估抗凝效果。采用SPSS v.28.0进行统计分析，显著性水平设为P

结果： 本研究共纳入100名患者，其中62%为男性，平均年龄为63 ± 14.5岁。导致AKI的主要原因是败血性休克、出血性休克和新冠肺炎（Covid-19）。ICU停留时间中位数为16天（RIC 8-43），院内死亡率为48%。治疗持续时间为60.5小时的中位数（RIC 38-107）。仅有一名患者出现出血情况，滤器凝结发生率占26%。没有报告柠檬酸钠毒性案例。电解质并发症包括低钙血症（45%）、低钾血症（41%）、低钠血症（36%）和代谢性酸中毒（30%）。

结论： 在使用柠檬酸钠的连续肾脏替代治疗中，机械性和代谢性并发症很常见。ICU工作人员必须意识到这些并发症的高发率以优化临床管理。
