Evaluation of a driving clinical decision pathway for generalist occupational therapists: Pilot test of practice change

背景: 目前几乎没有基于证据的资源可以帮助通用职业治疗师在实践中处理驾驶问题。本试点研究旨在评估一个驾驶临床决策路径是否可以协助通用职业治疗师解决客户面临的驾驶问题。
方法: 采用前后对照设计（Timepoint-1 和 Timepoint-2），数据是在多站点门诊社区康复服务中收集的。在两个时间点分别收集了医疗记录审核文档，这些文档记录了实践中如何处理驾驶问题，并且描述性调查了治疗师对路径使用的看法。实施了一个为期6个月的驾驶临床决策路径。使用描述性统计和内容分析来分析并比较不同时间段的数据。
结果: Timepoint-1 收集了来自102份客户医疗记录和13份治疗师调查表的数据，与Timepoint-2 的144份记录和8份调查进行了对比。在实施路径后，通用职业治疗师使用的评估数量增加了三倍，并且根据驾驶过程提出的建议也增加了一倍。治疗师自我报告的知识、技能和信心也增加了一倍。
结论: 一个全面的驾驶临床决策路径为临床医生提供了更多的结构和支持，以指导实践变革并促进在处理因健康状况变化而导致成年人重新获得驾驶能力的角色履行。
